Database & XML form

We want a php script that shows the current values in a MySQL database (an 8 x 32 table) inclusing text field and 2 radio buttons per option. It should be in the format of a web form so that users can change the values and update the database contents. We will provide a template.

When saved, the details should be saved in mysql and 2 xml files written from the values.
